Simeon Kwan, M.B.A., D.O.

Simeon Kwan, M.B.A., D.O., is a clinical assistant professor in the Department of Clinical Sciences at the Tilman J. Fertitta Family College of Medicine. He is board certified in both family medicine and hospice and palliative medicine and brings more than a decade of experience leading care for complex, high-risk patient populations across a variety of settings, including home-based, long-term, and institutional care.

Kwan currently serves as the West Regional Medical Officer for Carelon Health, where he oversees clinical operations and performance across the western United States. In this role, he manages regional medical directors, supports business development efforts, and leads policy and protocol development for palliative care services. He previously served as lead physician for the Texas and Louisiana markets and was an early contributor to Aspire Health, helping to shape the organization during its growth into what is now Carelon Health, a subsidiary of Elevance Health.

His career reflects a commitment to integrating compassionate, patient-centered care with strategic and operational leadership. Kwan has provided long-term and skilled nursing care in Houston, and served as medical director for the TOUCH/I-SNP program through CareMore Health, providing primary care services to geriatric patients in institutional settings. He also maintains a longstanding commitment to medical education as adjunct clinical faculty at the University of North Texas Health Science Center, where he teaches core geriatrics and palliative care rotations.

Kwan earned his medical degree from the University of North Texas Health Science Center and holds a master of business administration in healthcare management from West Texas A&M University. He completed his family medicine residency at Waco Family Medicine with a focus in international medicine, followed by a fellowship in hospice and palliative medicine at M.D. Anderson Cancer Center. He is a member of the American Academy of Hospice and Palliative Medicine, with multiple publications and presentations in the field of pain management and advance care planning.
